About the Role
We are seeking a Grade 2 Social Worker (0.4 EFT, two days per week – Mondays & Wednesdays) to join our Chemotherapy Day Unit (CDU) team at Moorabbin Hospital. In this rewarding onsite role, you will work alongside the CDU team and inpatient Social Work colleagues to:
- Triage new referrals and provide psychosocial and risk assessments
- Deliver emotional support for patients and families during treatment (onsite or via Telehealth)
- Collaborate with CDU staff, Palliative Care, and external care providers to ensure holistic support
You will manage a clinical caseload, applying evidence-based practice to deliver high-quality social work services. Your role will also include participation in quality, teaching, and research initiatives. A strong working knowledge of systems such as Family Violence, Child Protection, My Aged Care, TCP, NDIS, and Mental Health recovery models will be key to your success.
About Us
At Monash Health, Social Workers provide vital psychosocial support and advocacy through assessment, counselling, discharge planning, and connecting patients and families with community resources. As integral members of the multidisciplinary team, we support patients where social complexity exists. Services operate Monday–Friday, with additional rostered weekend and on-call coverage at selected sites.
About You
You are an experienced Social Worker ready to step into a specialised role, bringing:
- Demonstrated skills in complex psychosocial assessments
- Strong collaborative and communication skills within multidisciplinary care
- Interest in leadership, quality improvement, and research activities
- Commitment to AASW standards, ethics, and ongoing professional development
What You Need:
- Bachelor of Social Work, Masters of Social Work (Qualifying), or equivalent (AASW recognised)
- Eligibility for AASW membership
- Current Victorian Driver’s Licence
