About the Diabetes Unit 
The Diabetes Unit is a large multidisciplinary unit covering all 6  Monash Health sites integrating with Community Health. 
The aim is to provide integrated, transitional care across the community to acute sector continuum. Diabetes management is primarily focused on the ambulatory care setting and the unit now has 16 ambulatory care services across all ages and support needs. The Monash Health Diabetes Unit is located at both Dandenong Hospital and Monash Medical Centre in the Special Medicine Centre.

 

About the Role

This role is being offered to a GP registrar or vocationally qualified GP looking to develop their skills in diabetes management and build relationships with Monash Health. The role has been designed to provide the successful applicant extended hands on education and training in the management of complex diabetes under the supervision of a consultant endocrinologist. This is a fixed term position based at Casey suites (Berwick) which can be 6-12 months (by negotiation). The clinic runs weekly each Wednesday from 1.30PM until 5PM. 

The successful applicant will work alongside senior endocrinologist and Diabetes Nurse Educators (DNE) in managing a range of patients with diabetes and other comorbid conditions.  The successful applicant will see their own list of patients and be provided support from senior endocrinologists.  They will participate in a post clinic review meeting. 

With the increasing number of people being diagnosed with diabetes and the chronicity of the condition, there is a need to further enhance capacity of clinicians in primary care to manage more complex diabetes.  

 

What you need: 
•    Registration with the Australian Health Practitioner Regulation Agency (AHPRA) 
•    Appropriate health professional tertiary qualifications (RACGP)
•    Working with Children's Certificate
