Our Journey - Early in Life Mental Health Service (ELMHS)

ELMHS provides a range of mental health services to pregnant mothers, infants, children, adolescents, and their families within the Southern Region. The organisational and clinical model has been developed to provide mental health care that is flexible and responsive to the individual and developmental needs of the perinatal to adolescent populations and their families. As such, this model will support the provision of integrated mental health care for young people, guided by consistent, service-wide protocols and principles of care.

Please refer to Appendix B for a copy of the ELMHS Clinical Model.

About the Role - Team Manager, Core Community Treatment Team Adolescent Stream

The purpose of this position is to take on an operational leadership role as Team Manager of the Core Community Treatment Team within the Early in Life Mental Health Service with a particular focus on the adolescent population (12-17 stream). This role is open to various disciplines as per the position description; psychologists, speech therapists, registered nurses, social workers and occupational therapists with relevant experience and qualification are welcome to apply. The role will at times oversee the service for consumers and families in the under 12yr stream, depending on demand. The Core Community Treatment Team provides service-wide coordination of clinic-based individual, parent and ‘matrix’ treatment modalities. Matrix treatment refers to:

  • Care coordination
  • Longer-term treatment where significant psychiatric risk remains and/or
  • Multi-modal treatment that requires more than one clinician to implement.

There will be a strict clinical decision guideline implemented that will involve endorsement from the team’s Consultant Psychiatrist before a client can receive a Matrix treatment modality – ensuring that provision of other treatment modalities have been carefully considered. This process will support the integrity of separate treatment categories provided within the team. This redevelopment of the community treatment clinical model of care further supports treatment planning and provision that is responsive and flexible to the developmental and individual needs of young people. The reorganisation of clinical resources also supports the provision of evidenced-based, session-limited packages of care, that promote clear communication, partnerships and clinical review/discharge processes between young people, their families and the treating team.

This role will work very closely with the /Team Manager for the 0-11yo Stream, supporting each other across the 4 sites – Pakenham, Frankston, Berwick and Dandenong. This position will have oversight for 2 of the 4 sites.
