About the Role

Join our team as a Grade 3 Social Worker on a part time (0.6EFT) ongoing basis.  This position will see you joining the Casey Intesive Care Package (ICP) Team, providing assertive community based mental health assessment and treatment for consumers living in the Casey Adult Mental Health Services catchment area and currently engaged with the Pakenham Continuing Care Teams.

 

The Senior Social Work will work collaboratively with the members of the ICP team, which includes: Occuptional Therapy, Clinical Psychology, Dual Diagnosis, Peer Work, Art Therapy, along with the current treating team at Pakenham CCT.

 

About Us

The Casey Intensive Care Package Team is made up of experienced mental health clinicians offering speciality support to consumers with identified intensive mental health needs in the community. We provide care consistent with the Intensive Community Mental Health Packages guidelines from the Victorian Department of Health. 

 

This multidisciplinary team has been developed to The ICP is a dynamic and innovative team, supporting consumers in the Casey/ Cardinia catchment, who are wanting and needing more support, than what case management can provide. The team provides 1:1 treatment and intervention, as well as a group program, offering a range of skill development and areas of interest for consumers.

 

 About you

The incumbent for this position is a Social Worker with a minimum of 7 years post graduate experience, with established skills in assessing and collaborating with consumers who are currently linked with our mental health program case management services.

 

You will be able to showcase your skills in ability to identify and plan intervention for complex needs as identified by the consumer, carers and service providers. These consumers will have a history of frequent readmission and presentations to acute services requiring a multidisciplinary approach to identifying and supporting the development of recovery goals and supporting the consumer and their existing treating team to implement these recommended interventions.

 

Education & Licence requirements are:

  • Bachelor of Social Work or Masters Social Work Qualifying OR Master of Social Work, OR equivalent as assessed and approved by the Australian Association of Social Workers (AASW)
  • Eligible for ordinary membership of AASW (http://www.aasw.asn.au/)
  • Written evidence of eligibility for AASW membership for overseas qualifications is required.
  • Accredited Social Worker and/or Accredited Mental Health Social Worker: As approved by the AASW
  • Completion of a post graduate qualification in relevant field or working towards
