About the Role
The Subacute Intake Coordinator will service the broader Adult Mental Health Program, whose focus will be on facilitating and enhancing patient flow and timely transitions of consumers across the program. The roles incumbent will be excellent in communication, organisation and collaboration to ensure success.
The key purpose of the role is to provide a central intake process for referrers and ensure smooth consumer transitions between Emergency Departments, Consultation Liaison Team, Mental Health Acute wards and Community Mental Health Services into the residential CCU, PARCs and SEPH .
The role is also responsible to assist the Mental Health Bed Coordinator, including patient flow and bed management for the Mental Health Program.
A Day in the Life
Ensuring daily overview of patient flow, bed requests, liaison with bed managers, NUMs and community managers, the Subacute Intake Coordinator will be monitoring, identifying and coordinating the flow of patients into the mental health program subacute areas. Using the clinical eligibility criteria for
each area, the clinician will collaborate with each site to ensure timely communication to facilitate access to appropriate subacute services and optimisation of utilisation of subacute resources.
What you need:
- Able to meet the Person specification requirements as outlined in the Position description below
- Understanding of emergency, acute and subacute mental health services an advantage
- Demonstrated experience within an Inpatient Unit or similar
- Current AHPRA registration
