About the Role
As an Aged Care Assessor, you will work closely with seniors and their families to assess their care needs, ensuring they have access to appropriate services and resources. Your role will directly impact the well-being of older individuals, helping them live with dignity, safety, and comfort.
Key Responsibilities:
- Conduct thorough assessments of older individuals to determine their care needs, preferences, and eligibility for aged care services.
- Work with clients, families, and care providers to develop personalized care plans that support the individual's physical, emotional, and social needs.
- Ensure that all assessments are completed in accordance with current legislation, policies, and quality standards.
- Collaborate with other health professionals, government agencies, and service providers to coordinate services and ensure holistic care.
- Advocate for the rights of older adults and ensure their voices are heard throughout the assessment and care process.
- Monitor and review care plans to ensure they remain relevant and effective in meeting the changing needs of clients.
About you:
We are seeking individuals who are passionate about aged care and public health. The ideal candidate will have:
- Experience or qualifications in Aged Care, Nursing, Social Work, or a related field.
- Appropriate qualifications
- Proven clinical experience
- Strong communication, empathy, and assessment skills.
- Knowledge of aged care services, policies, and regulations.
- A commitment to delivering person-centered care and advocating for older adults' rights.
- Ability to work independently and as part of a multidisciplinary team.
