About Us:
This is a Fixed Term position, covering Annual Leave from 30 December 2024 to 28 April 2025 (inclusive). Both full and part time applications will be considered.
The Monash Health Community Rehabilitation team is looking for a Grade 2 Allied Health Professional or Community Nurse to join our Kingston and Cranbourne home based teams on a temporary basis (annual leave cover). This Case Manager role presents an excellent opportunity to broaden your skills and gain experience in the Community setting, and will be well suited to an experienced Grade 1 or a Grade 2 Clinician looking for a change.
This position is responsible for the care of adult clients in Community Rehabilitation across a variety of clinical streams, including neurological, musculoskeletal, general, cardiac and pulmonary rehabilitation. The team is well supported with a Senior Clinician Case Manager and Team Leaders.

What you need:
- Allied Health qualification with minimum 4 years’ experience or experienced Registered Nurse with 4 years post grad experience
- Current registration with APHRA or Registered with Nursing and Midwifery Board of Australia via AHPRA
- Eligible for membership of relevant discipline specific professional body
- Relevant professional experience working with adults in a rehabilitation, community or case management setting
