About our Community Mental Health Service at Older Adult Mental Health and Wellbeing

 

The Aged Persons Assessment and Treatment (APAT) Team provides community treatment to clients who are generally over 65 years old, have a mental illness and live in the Middle South catchment area. The aim of the APAT Team is to provide treatment to clients in their own environment as far as clinically possible for a time limited period and to connect clients into relevant community services including primary care. APAT Team play a pivotal role in preventing relapse and admissions whilst providing treatment in the least restrictive manner. APAT works as part of the wider Older Adult Mental Health and Wellbeing Community Team. The APAT Mental Health Nurse provides a range of clinical mental health services, conduct intake, mental state examinations, risk assessments, care planning, treatment, case management and discharge planning.


About The Role 

 

As a Mental Health Nurse you will work collaboratively with clinicians of various disciplines to ensure high quality of care is provided to consumers in the service area. We will look to you to provide comprehensive mental health assessments and demonstrate competence in managing complex, mental health issues whilst adjusting to changing priorities. This role provides an excellent opportunity for aspiring clinicians to explore community mental health or as an opportunity to try Older Adult Mental Health, extensive training is provided where required. 

 

  • This position is Part-time up to 4 days a week (32 hours) covering parental leave until March 2026

Your approach is patient centred and are confident working in a small team and independently.

 

What You Need:

 

•    Relevant Qualification (aligned to position description)
•    Demonstrated experience preferably in community mental health or equivalent
•    Working with older adults (65+) is highly desirable
•    Current AHPRA registration
