About The Role
The Aboriginal Access and Support Worker provides culturally safe, holistic support to Aboriginal and Torres Strait Islander people aged under 50, enabling improved access to Monash Health services across hospital and community settings. This role delivers a home and community-based care program that supports clients and their families through practical assistance, advocacy, and culturally informed care.
Working closely with internal and external health professionals, Aboriginal Controlled Organisations and community services, the role contributes to coordinated care planning and improved health outcomes. Support may include transport assistance, outreach, navigation of outpatient appointments, and assistance accessing services such as My Aged Care and the NDIS.
The Aboriginal Access and Support Worker also plays a key role in promoting the program within the Dandenong and Casey communities, strengthening relationships, and providing secondary consultation to non-Aboriginal staff to promote culturally safe practice. Accurate and timely documentation across Monash Health systems (HMS, SMR, EMR and other data management systems) is a core requirement of the role.
This position is central to supporting the cultural, emotional and spiritual wellbeing of Aboriginal and Torres Strait Islander people, ensuring their voices are heard and their health care journey is supported across the lifespan.

The Aboriginal Health and Engagement Directorate supports the health and wellbeing of Aboriginal and Torres Strait Islander patients across hospital and community settings, including the Aboriginal Hospital Liaison Service and Aboriginal Health Services within Monash Health Community.
About You
This position is identified and is open only to Aboriginal and/or Torres Strait Islander people. Applicants must identify as Aboriginal and/or Torres Strait Islander, demonstrate acceptance by their community, and actively participate in Aboriginal cultural activities.
Essential Qualifications & Requirements include:
- Certificate IV in Aboriginal and Torres Strait Islander Primary Health Care (Community Care) or equivalent
- Current Victorian Driver’s Licence.
You will bring the follwoing skills, knowledge & experience:
- Minimum of two years’ experience working in the community health or related sector.
- Demonstrated experience working with Aboriginal and Torres Strait Islander communities, with a strong understanding of culture, customs, lore and kinship.
- Sound knowledge of the mainstream health care system across hospital and community settings.
- Demonstrated understanding of person-centred, strength-based practice and client self-empowerment approaches.
- Strong written and verbal communication skills.
- Well-developed organisational and time management skills.
- Experience using electronic referral, care planning and patient data management systems, with the ability to accurately collect and enter data.
- Flexible, adaptable and open to change, with the ability to contribute positively to team development.
- Highly motivated, with a willingness to learn and adapt to evolving roles and relationships.
