About the Role:
Monash Health is launching a pilot for Acute Medical Transition Unit (AMTU) within the General Medicine Program at Monash Medical Centre in Clayton for 0.4EFT including weekends and extended business hours. Designed for higher‑acuity, frail or complex patients requiring rapid assessment and early supported discharge, the AMTU will operate extended hours across seven days, ensuring timely, senior-led decision‑making and patient flow.
We are seeking an experienced Grade 3 Allied Health clinician (Exercise Physiology, Nutrition & Dietetics, Occupational Therapy, Physiotherapy, Podiatry, Social Work or Speech Pathology) to join AMTU as the Trans Professional Lead.
As a pilot role, the position will evolve as the model is tested and refined. This project is till June 2027. The successful applicant will work extended hours and weekends, aligning with the AMTU’s seven‑day service model.
Working closely with nursing, medical, pharmacy, and allied health colleagues, you will play a key role in supporting safe, effective, and timely hospital discharge.
You will conduct comprehensive trans-professional assessments and interventions, and arrange necessary follow-up and referrals to enable patients to return home or access appropriate community-based services. A major focus of this role is enhancing weekend allied health service capacity to improve patient flow, reduce length of stay, and minimise unnecessary time in hospital.
Key responsibilities include:
- Providing senior Allied Health decision-making at the front end of care
- Delivering comprehensive trans professional assessments and interventions
- Leading Allied Health involvement in patient flow, rapid discharge planning and community transitions
- Supporting the co‑design, implementation and evaluation of the AMTU model, in partnership with the clinical leads and Transformation Office
- Refining Allied Health workflows using IHI Improvement Science methodology
- Building strong internal and external relationships to streamline care pathways
About You
The successful candidate will need at least 7 years of acute clinical experience in your professional discipline. Have expert skills in assessment, care coordination and discharge planning. They will have strong clinical reasoning and capacity to work independently in a fast‑paced pilot environment. Demonstrated leadership capability and experience supervising staff or students, plus the ability to adapt and thrive in an evolving model of care.
Trans professional practice experience is desirable.
