About the Role
Please join us in an exciting new pilot role within Monash Care in the Home – The Monash Health Older Persons Care Hub (OPCH).
Are you an Administration Officer wanting to make a difference to Older Australians over 65?
The Monash Health Older Persons Care Hub (OPCH) is a non-admitted, multi-disciplinary early intervention model of care providing a home-based alternative to Emergency Department (ED) or hospital presentation for older people living in the community. Operating within the Monash Care in the Home Directorate. The service targets older people living at home at risk of Emergency Department presentation, following referral from the Victorian Virtual Emergency Department (VVED), Ambulance Victoria and other pathways. The OPCH is a multi-disciplinary team comprising Geriatricians, Nursing, Allied Health and Administrative staff.
The team will focus on:
- Comprehensive screening, assessment and referral to appropriate Monash Health and external services.
- Short-term intervention to address immediate needs to avoid hospital presentations.
- Supporting patients to access the right care at the right time, reducing unnecessary hospital admissions and improving continuity of care.
Allied Health, nursing and medical staff will provide face-to-face visits or telehealth consultations, as appropriate. Medical governance will remain with the treating GP, and once care is provided, clients are discharged back to the GP’s care
Role Overview
The successful applicant will be responsible for:
- The provision of efficient and effective clerical support to patients, clinical and project staff.
- Co-ordination in relation to administrative support, including admissions, discharges and appointment planning.
- Displaying competent technical skills and a professional attitude.
- Support Nurse Manager by liaising with internal and external partners to ensure appropriate referral documentation is processed as required.
- Support OPCH optimisation work as requested by Nurse Manager.
- Additional data entry of patient information into OPCH reporting templates as required.
- Maintain the Monash Health visions and values.
About You
To thrive in this role, you will bring the following:
- Strong interpersonal and communication skills, both written and verbal.
- Demonstrated excellent problem solving and organisational skills
- Data entry processing and keyboard skills.
- Strong interpersonal, time management and communication skills
- Proficiency in use of Microsoft Office suite of programs
- Experience in general administrative roles and customer service
Desirable:
- Previous experience in acute geriatric services, hospital substitution models, or acute/community interface services
- Knowledge & experience working with the Health Management System - HMS
