About the Role:
Join our team as an Advanced Allied Health Geriatric Practitioner on a full-time 12-month, fixed-term contract, where you will have opportunity to work in Monash Health’s fast paced Emergency Departments across Dandenong and Monash Medical Centre Clayton (required to work some evening and weekend shifts).
Are you looking to change work areas to lead innovative interdisciplinary care for geriatric patients within the Emergency Departments at Dandenong Hospital and Monash Medical Centre, Clayton?
If so, then a fantastic opportunity exists for enthusiastic, motivated and dynamic Allied Health and Pharmacy professionals (Grade 4 Dietitian, Occupational Therapist, Pharmacist, Physiotherapist, Podiatrist, Social Worker or Speech Pathologist) to join the Timely Emergency Care Collaborative (TECC) project team, working closely with the Emergency Department Care Coordinators and Geriatiricians. These two fixed term positions (for a maximum of 12 months, ending no later than June 2026) will be responsible for designing, implementing, and evaluating Allied Health interventions targeting adults aged over 65 years, and Aboriginal and/or Torres Strait Islander People aged over 50 years aimed at minimising functional decline and frailty.
Key tasks will include:
- Working with the project team and clinical experts to develop and implment a targeted needs assesssment for patients who are at risk of functional decline / frailty
- Co-design a new Advanced Practice Trans-professional Geratric Allied Health Practitioner role
- Implement and evaluate the Advanced Practice Trans-professional Geratric Allied Health Practitioner role(s)

About You:
To ensure your success in this role you will bring the following demonstrated skills, experience and knowledge:
- Extensive experience in in the provision of care to older people to achieve best practice clinical standards, including complex systems navigation and discharge planning
- Advanced knowledge in the application of evidence and theory to practice in the area of Gerontology
- Leadership experience in innovative approaches to problem solving for individual consumers and/or service delivery
- Experience working in a transdisciplinary Allied Health team, including experience performing interprofessional skills within endorsed scope of practice
- Previous experience working in an Emergency Department would be beneficial
On a personal level, you will demonstrate:
- Advanced level of recognised clinical leadership experience in a gerontology services including service development and leading others through change
- Ability to work autonomously and collaboratively with the multidisciplinary team to develop and deliver safe, consumer focused care
- Previous experience leading both formal research projects and quality improvement activities
Mandatory requirements of the role:
- A relevant tertiary qualification in the related Allied Health profession
- Current unconditional AHPRA registration (for Occupational Therapists, Pharmacists, Physiotherapists and Podiatrists)
- Post graduate qualification, or work towards, in the field of geriatric management is desired
