About the Role:
Join our team as an Advanced Allied Health Geriatric Practitioner and make a real impact in Monash Health’s fast-paced Emergency Departments. This is a full-time or part-time fixed-term role (until June 2026), with some evening and weekend shifts required.
Are you looking to change work areas to lead innovative interdisciplinary care for geriatric patients within the Monash Health Emergency Departments?
If so, then a fantastic opportunity exists for enthusiastic, motivated and dynamic Grade 4 Allied Health and Pharmacy professionals (Dietitian, Exercise Physiologist, Occupational Therapist, Pharmacist, Physiotherapist, Podiatrist, Social Worker or Speech Pathologist) to join the Timely Emergency Care Collaborative (TECC) project team, working as part of the Acute Frailty Team in collaboration with the Emergency Department Geriatiricians, Care Coordinators and the broader ED team. You will be responsible for implementing and supporting the evaluation of Acute Frailty Team initiatives, with a focus on avoiding hospital admissions and minimising acute length of stay for frail older persons.
Key tasks will include:
- Working with the project team and clinical experts to develop and implment a targeted needs assesssment for patients who are identified as frail
- Provide early intervention in the Emergency Department and Short Stay Unit aiming to avoid hospital admissions using a home first approach
- Utilise expertise in Geriatrics and Frailty, and extensive specialist knowledge of home based, community and residential services to navigate complex service systems to provide exceptional care to older people, ensuring their care is provided at the right place and right time.

About You:
To ensure your success in this role you will bring the following demonstrated skills, experience and knowledge:
- Extensive experience in in the provision of care to older people to achieve best practice clinical standards, including complex systems navigation and discharge planning
- Advanced knowledge in the application of evidence and theory to practice in the area of Gerontology
- Leadership experience in innovative approaches to problem solving for individual consumers and/or service delivery
- Experience working in a transdisciplinary Allied Health team, including experience performing interprofessional skills within endorsed scope of practice
- Previous experience working in an Emergency Department would be beneficial
On a personal level, you will demonstrate:
- Advanced level of recognised clinical leadership experience in a gerontology services including service development and leading others through change
- Ability to work autonomously and collaboratively with the multidisciplinary team to develop and deliver safe, consumer focused care
- Previous experience leading both formal research projects and quality improvement activities
Mandatory requirements of the role:
- A relevant tertiary qualification in the related Allied Health profession
- Current unconditional AHPRA registration (for Occupational Therapists, Pharmacists, Physiotherapists and Podiatrists)
- Post graduate qualification, or work towards, in the field of geriatric management is desired
