We have an exciting opportunity for an experienced Allied Health Assistant Grade 3 to take a Permanent Full-time role in the Aged and Community Care Team. This position will support the team by completing individual, home visits and having opportunities to run group programs (For example: Falls prevention, strength and conditioning groups).  

 

Our Journey

The Aged and Community Care (ACC) Program provides an interdisciplinary service to support older people in the community. The program aims to keep the client independent within their community, proactively promoting health, independence, and self-management whilst assisting with timely interventions to prevent deterioration of the condition, and admission to hospital or residential care. We offer a client- centred, goal directed approach to a growing and diverse population.

 

A Day in the Life 

Our team of highly experienced Health Professionals work to provide timely, person-centred interventions to prevent deterioration of health, and admission to hospital and or residential care. This is a unique opportunity to join our large, friendly community-based team, with a highly varied caseload (where no two days will be the same!). You will be part of the wider Monash Health Allied Health Assistants network with access to all benefits including in- house professional development, supervision, and broad range of clinical services.

What you need:  

  • Certificate IV Allied Health Assistance  
  • Ideally greater than 3 years AHA Experience (desirable) 
  • Current Driver’s License
