Our Journey

Monash at Home – Aged and Rehabilitation Care (ARC) is a dynamic service that delivers care to patients within their own home.  The team consists of Medical, Nursing, Allied Health, Pharmacy, Psychology and Administration staff.  Monash at Home (ARC) provides an alternative to inpatient care, and providing safe and high quality home based services for individuals with complex medical, functional and cognitive issues, who require comprehensive, medically led multidisciplinary assessment and intervention within the comfort of their own home.  Monash at Home ARC is a 40 bed services that covers the Monash Health catchment.  The service operates out of two sites, Kingston Centre (Heatherton) and Cranbourne, and staff are required to work flexibly across both sites to meet patient needs.

 

Patients within the service receive a daily visit from nursing.  Medical, allied health, neuropsychology and pharmacy also provide face to face visits or Telehealth which are dependent on patient needs.  Patients also have access to 24 hour telephone support.The service also aims to provide short term, high quality, safe and effective patient centred care to patients within the primary Monash Health catchment.  

 

The Allied Health Assistant (AHA) works within a defined scope of practice, and the role involves conducting face to face, and/or Telehealth intervention to patients who are admitted to the service under the delegation of an Allied Health Professional (AHP).  The AHA will receive delegation or supervision from the Allied Health Professional (AHP) through direct, indirect or remote supervision.  The Allied Health Assistant is required to provide best practice patient care, achieve maximal operational effectiveness and support Monash Health's Strategic Guiding Principles.

 

We are looking for a dynamic Grade 3 Allied Health Assisstant to join our innovative team on a part time basis to support our patients to receive the best clinical care at home. Business hours are Monday-Fridays 0800 – 1630 hrs.

 

A Day in the Life

The Grade 3 Allied Health Assistant will actively contribute to:

  • Providing a high standard of evidence based care and intervention, under the supervision of an Allied Health Professional, to facilitate safe, effective and person-centred care
  • Participating in the development of a collaborative inter-professional team approach to achieve optimal patient care and outcomes
  • Creating strong partnerships within the Monash at Home interdisciplinary team in collaboration with the Allied Health and Psychology Manager, Allied Health Professionals, Nurse Manager, Clinical Coordinators and other team members
  • Assuming responsibility for relevant administrative portfolios as directed by the Allied Health and Psychology Manager or delegate

 

About you

  • You are a dynamic Allied Health Assistant with a genuine passion and interest in working with patients within a home environment
  • You have experience in working within a healthcare setting
  • You have proven ability to work both independently and collaboratively and readily build strong working relationships with key stakeholders in the delivery of client centred care
  • You have excellent communication and interpersonal skills

 

What you need

  • Certificate IV Allied Health Assistance and have completed a minimum of 3 years of experience as a Grade 2 (or equivalent)
  • Eligibility for membership of AHANA as a Practicing Member (Certified)
  • Australian working rights
  • Valid Driver's licence with no restrictions
