About the Role

Join our team as a Clinical Nurse Consultant on a full time ongoing to work at Cardiac Ambulatory Care based at Victorian Heart Hospital Clayton

 

The Clinical Nurse Consultant (CNC) Team with the Ambulatory Services Program are centrally located and supported by an administrative team. The Clinical Nurse Consultant team supports the cardiology streams for Heart Failure, Chest Pain, Structural Heart, and Cardiac Rhythm Management. Each CNC stream works closely with not only the multidisciplinary team from their designated stream, but in partnership with the other streams to ensure that patient receives optimised patient outcome and service delivery. Following a recent review of the Ambulatory Service and the roles of the CNC team, we aim to explore CNC responsibilities, identifying areas of enhanced efficiency, and aligning roles with best practice.

Our Ambulatory Cardiac Services is one of the largest programs in the Victorian Heart Hospital delivering an innovative model of care and a comprehensive range of acute and chronic cardiac consulting services. Supported by a new purpose built EMR module, our Ambulatory Service includes Referral management, bookings and scheduling for ambulatory clinics, ambulatory testing, preadmission, diagnostic procedures, and support for Multidisciplinary Team Meetings. Our Ambulatory Cardiac Services aims to be a ‘one stop shop’ providing patients with an efficient, streamlined, and integrated approach to the patient journey within a multidisciplinary team focus, with vastly reduced duplication and patient movements, and standardisation of care

 

About You

The Cardiac Rhythm CNC is a diverse role, in the outpatient setting, but also involved with inpatients and research. Reporting to the Director Clinical Operations (DCO) Cardiac Ambulatory Services, the CNC role is multifaceted senior role combining clinical leadership and expertise to streamline clinical services planning with education, research, and management to triage patients. A specialised role in nurse led Atrial Fibrillation care, whilst ensuring the rhythm clinics operate as a streamlined, patient centred service, with timely management of patients referred to the service. The role involves coordination and general oversight of the cardiac rhythm related research trials under the supervision of the Monash Cardiovascular Research Centre (MCRC) research manager and ensures the successful commencement and completion of relevant CRM based commercial clinical trials and investigator related research educated by Head of CRM and Director of MCRC

 

You have strong management and leadership skills combined with clinical expertise and takes personal pride in providing excellence in patient centred care. You are dedicated to supporting complex patients and thrive in leading by example in the pursuit of excellence to collaboratively drive change and positively influence patient outcomes. To ensure your success in this role, you will have the following demonstrated skills, experience, and knowledge

 

  • AHPRA Registration
  • Registered Nurse (current registration with Australian Health Practitioner Regulation Agency
  • Post graduate qualification in Cardiac nursing
  • Demonstrated senior experience in Cardiac nursing
  • Ability to operate under Good Clinical Practice (GCP) principles in a highly ethically and responsible manner.
  • Substantial experience in cardiology clinical research
  • Previous involvement and experience of cardiology research or clinical trials including demonstrable knowledge and awareness of the National Statement on Research and Good Clinical Research Principles
  • Extensive experience in acute cardiac management.
  • Extensive exposure and understanding of a multidisciplinary cardiology unit.
  • Demonstrated ability and high standard in cardiac rhythm management.
  • Demonstrated ability and high standard in interpretation of ECGs
  • Demonstrated current and wide-ranging knowledge in latest trends, equipment, and issues in the cardiac rhythm field
  • Previous experience in cardiac education
  • Current knowledge of health systems, funding and performance issues
  • Outstanding leadership skills and engage with diverse stakeholders to drive outcomes
  • Proficiency in computer software programs
