Monash Health Emergency Department Care Coordinators are an integral part of the dynamic multi-disciplinary team providing care across our three Emergency Departments at Casey Hospital, Dandenong Hospital and Monash Medical Centre. As a valuable member of a wider team, we are committed to working with you to become your professional best. About the Role:
Join our team as a Care Coordinator Grade 2 and contribute to the dynamic, fast-paced environment of Monash Health’s Emergency Departments. This is a part-time, fixed-term opportunity (0.30 EFT / 22.8 hours per fortnight) available until June 2027.
This role requires availability to work at least one weekend per fortnight.
A fantastic opportunity for an enthusiastic, motivated and dynamic Allied Health professional (Grade 2 Occupational Therapist, Physiotherapist, Podiatrist, Social Worker or Speech Pathologist) to join the Emergency Department Care Coordination teams working within the 3 Monash Health Emergency Departments - Clayton, Dandenong and Casey (Berwick) as an ED Care Coordinator.
Key responsibilities include:

Providing a high standard of client assessment, treatment and coordination of services to patients, to facilitate safe, effective, and person centred care

Providing assessments, treatments and interventions of an interdisciplinary nature in line with the competencies that have been developed for these roles

Participating in the development of a collaborative inter-professional team approach to achieve optimal patient care and outcomes

Actively challenging traditional models of care, with the aim of delivering effective clinical outcomes in the most efficient manner

Work with Team Leader, Allied Health staff and Emergency Department staff to ensure workload within the team is coordinated, prioritised and fairly distributed across the team and maximises operational effectiveness
About You:

Bring a range of clinical experience

Excellent problem-solving skills and a positive, can-do attitude

The ability to work autonomously is essential

Experience in working in acute healthcare is preferred

Previous experience working in an Emergency Department would be beneficial
Mandatory Requirements:

A relevant tertiary qualification in the related Allied Health profession

Current unconditional AHPRA registration
-
Membership of relevant professional Association and relevant Special Interest Group(s) desired
