Our Journey
Monash Health Gastroenterology is the largest service of its kind in Victoria, delivering over 18,000 outpatient consultations annually across a broad range of specialty clinics, including a high-volume Inflammatory Bowel Disease (IBD) service. The IBD team offers specialist multidisciplinary care across five dedicated IBD clinics, including young adult and pregnancy-focused services, and supports a significant cohort of patients on advanced biologic therapy.
The IBD service is integrated with clinical trials, research, colorectal surgery, endoscopy, and patient helplines to support comprehensive, person-centred care. This is an exciting opportunity for an experienced and driven CNC to step into a dynamic and collaborative team during a period of growth and innovation.
A Day in the Life
As the IBD CNC, you will be a pivotal part of the multidisciplinary team, using your advanced clinical and theoretical expertise to support the care of patients living with complex inflammatory bowel diseases.
You will:

Provide expert assessment, triage, and education to IBD patients across multiple outpatient clinics

Facilitate and lead nurse-led clinics and provide frontline support via the IBD Helpline

Coordinate and interpret diagnostics and investigations to support timely care
-
Work closely with IBD consultants, colorectal surgeons, pharmacists, and research nurses
-
Ensure access to biologic therapies and participate in virtual biologic clinics
-
Oversee capacity and demand across IBD outpatient clinics and streamline access
-
Lead and support IBD-focused health promotion, patient education, and clinical transition
-
Contribute to and coordinate IBD multidisciplinary team (MDT) meetings
-
Support IBD patients transitioning from inpatient to outpatient settings
-
Engage in clinical trials and compassionate use programs where appropriate
About You
You are an experienced, patient-centred nurse with a passion for gastroenterology and chronic disease management. You’re a strong communicator, an autonomous practitioner, and a collaborative team player. You lead with compassion and evidence-based practice and are committed to advancing the care of IBD patients.
You will also demonstrate:

Proven experience in IBD, gastroenterology or similar specialty nursing role

Ability to assess, manage, and coordinate complex clinical care independently and in collaboration with medical staff

Strong communication, negotiation, and patient education skills
-
Capacity to work in a fast-paced environment and contribute to service development
-
Ability to use clinical databases and contribute to service performance improvement
What You Need
-
Current registration with the Nursing and Midwifery Board of Australia via AHPRA
-
Minimum of five years post-registration nursing experience, including in a specialty or advanced practice role
-
Experience working in gastroenterology or with IBD patients
-
Postgraduate qualification in Gastroenterology or relevant clinical field (desirable)

Understanding of the Victorian Specialist Clinic Access Policy

Working knowledge of complex and compassionate drugs access programs
-
Current National Police Check and Working with Children Check
