About the Role
The Clinical Nurse Consultant- Oral Haematological Chemotherapy Care Pathway is an experienced senior Clinical Cancer Nurse working collaboratively between the Haematology Units at Monash Health and Peninsula Health. The Clinical Nurse Consultant will receive referrals from across both Monash Health and Peninsula Health. The Clinical Nurse Consultant – supports Haematology patients with myeloid blood cancers on oral anti-cancer therapies utilising their advanced clinical assessment skills to assist patients with symptom management. The Clinical Nurse Consultant will act as a resource to patients, providing ongoing education and emotional support, assessing for adherence concerns and toxicities, and recommending strategies to improve compliance and self-care. Participates in the tracking and monitoring of metrics and outcomes, in collaboration with project lead, will document and evaluate outcomes of the program and report findings to relevant working groups.
- Develop and promotes pathways for patient recruitment by collaborating with internal and external stakeholders.

- Implement the optimal care pathway
- Contributes to the knowledge base of the health care community and in support of the role through activities such as involvement in professional organizations, presentations, publications, and research.
- Undertake administration tasks inline with the requirements of the project.
- Development of reports at pre-determined timelines to the committee.
- Discuss feasibility of having join MDM sessions with Peninsula Health and Monash Health.
- Implement the optimal care pathway
- Conduct audits to ensure the project is meeting the key reporting requirements of the final project report
The role will also involve project management tasks developing, implementing and evaluating this pilot under the guidance of the project committee. The Clinical Nurse Consultant and project committee will explore options / platforms / tools that may be utilised to streamline processes between both health services

About You:
- AHPRA registration
-
Post graduate qualifications in relevant cancer speciality

Significant experience in Haemaotlogy, Oncology cancer care

ADAC Modules 1-7 plus attendance at workshop.

A proven commitment to the principles of cancer care and evidence based practice

High level of computer literacy

Ability to work across multiple Monash Health sites as required
