About Us
Monash Health Supportive and Palliative Care Service provides care for patients with a life limiting illness, incorporating a 16 bed inpatient palliative care unit located at Monash Medical Centre Clayton, a 5 bed inpatient palliative care unit at Casey Hospital and a Palliative Care Consultancy service.
The Palliative Care Consultancy service is a bi-disciplinary team consisting of both Clinical Nurse Consultants and Palliative Medical Registrars and Consultants who provide palliative care expertise within the acute care setting. We are at the forefront of change within Palliative Care. Teaching, education and research play an important role and our affiliation with Monash University enables us to readily access significant research undertaken in the area. We are currently seeking experienced Clinical Nurse Consultants to join our established team. We have 0.8EFT available on an ongoing basis, and 0.8EFT available for a 6 month contract. The service operates Monday to Friday, 8am – 4.30pm with days negotiable and is based across all Monash Health sites.
About the Role
You will provide best practice care to clients and their care givers. You will provide education relating to symptom management and will assist client to establish their goals of care, in collaboration with the wider multidisciplinary team. You will also provide education and support to ward staff regarding care of the palliative patient.
With your strong time management skills, you will effectively manage your workload and complete assessments in a timely manner. This role requires you to possess high level interpersonal and communication skills to assist with formulation and coordination of appropriate discharge plans in collaboration with the multi-disciplinary team, initiate referrals and regularly liaise with community service providers.

- Highly developed interpersonal and communication skills
- Willingness to provide education and support to patients, carers and ward staff
- Demonstrated knowledge of the palliative care service
- A proactive and team player attitude
- A proven commitment to the principles of palliative care and evidence-based practice
- Willingness to participate in quality and research initiatives within the nursing team and broader Palliative Care Service
You will also possess:
- Current AHPRA registration, essential
- Significant post grad experience within Palliative Care Services, essential
- Post graduate studies within a related field, essential
- Current Victorian Driver’s License, essential
