Our Journey:  

Primary Care Support Team, as part of the Community Assessment and Response Directorate offers a variety of programs that assist individuals to manage chronic health conditions, covering Greater Dandenong and Cardinia Casey, focusing on the assessment and delivery of client focused care.  

The team consists of Allied Health Assistants, Care Coordinators, Community Development Workers, Counsellors, Diabetes Nurse Educators, Dietitians, Exercise Physiologists, Psychologists, Occupational Therapists, Physiotherapists, Recreational Therapists, Social Workers, and Podiatrists. The team is committed to working with communities, families and individuals who have poor health outcomes and to providing high quality, multidisciplinary programs which embrace client centred care, evidence based practice and the Social Model of Health.  

We are looking for a Care Coordinator on a part time, 0.6EFT (3 days per week) fixed term basis until 28/02/2026. 

 

A Day in the Life:  

Monash Health Primary Care Support is a dynamic service with dedicated, highly skilled multidisciplinary team. The Care Coordinator will provide individual evidence-based assessment and intervention to clients with a wide range of clinical presentations, often presenting with multiple co-morbidities. The successful candidate will work in collaboration with the multidisciplinary team and be responsible for holding a clinical caseload which will involve assessment, care coordination, systems navigation, secondary consultation, service development and innovation.

What you need:  

  •  Current and ongoing employment screening, including but not limited to, National Police Check, employee Working with Children Check (WWC) 

  • Appropriate professional registration/affiliation with peak body
  • Demonstration of appropriate competencies and skills  

  • Current Victorian Drivers Licence 

  • Tertiary qualifications in Nursing, Allied Health or equivalent
