About Monash Children’s
We care for more than 100,000 children every year.
Monash Children’s Hospital is part of Monash Health, Victoria’s largest healthcare service and one of only four accredited Academic Health Science Centres in Australia. We have access to the world’s leading medical minds which are turning research findings into better health outcomes for our patients.
Monash Children’s Hospital is a network of paediatric healthcare services across three sites – our main campus in Clayton (co-located with Monash Medical Centre) and satellite sites at Dandenong and Casey Hospitals.
Monash Children’s Hospital is also the Victorian Referral Centre for many low-volume and high-complex cases. Our services are linked to an adult service, allowing us to provide transition-of-care as children grow older and move to an adult service.
We deliver specialised, innovative, family-centred care and believe strongly in putting patients first and striving for exceptional care and outstanding outcomes.
About Monash Children’s @ Home
Monash Children’s @ Home (MC@H) medical oversees several services.
-
HITH Hospital in the Home – neonatal/paediatric
-
PACS Post Acute Care Service
-
Complex Kids Run in conjunction with the RCH Complex Care Hub.
-
School programme
-
IHP In home programme (long term, e.g. CP)
HITH provides acute hospital care in the comfort of the person's own home or other suitable environment, as an alternative to staying in a hospital bed. The patient must have a medical condition that can be safely managed in the community setting, with patients admitted under the appropriate specialty bedcard unit and if appropriate direct from ED/outpatients after review by MC@H ATR. Generally the setting for care is the patient’s usual home environment, or Monash Health sites including Pakenham, Cranbourne, Clyde, Casey, Dandenong, and Clayton.
The care is provided through nurses, doctors and allied health professionals. It is available to all hospitals within Monash Health with programs also available across most of Victoria. As a multidisciplinary unit it is a unique opportunity to collaborate to meet patient care needs holistically.
Nursing staff visit patients in their own home, maximum twice per day for up to one hour per visit. Patients receive medical review weekly or as required.
The case-mix of patients includes ages 0 – 18, with conditions across the spectrum of medical and surgical specialities. Our unit provides care for a high number of neonates as Monash Health has multiple maternity and neonatal units across its sites.
PAC provides a shorter duration of post hospital nursing visits for less complicated patient requirements.
Complex Kids provides a home based programme for children with high medical needs and intensive support requirements. It includes support for children on home ventilator support, PN, PEG/J fed, tracheostomy. It can provide carer training, equipment, nurse liaison, care coordination, and visits.
