About the Paediatric Diabetes team
The Paediatric diabetes service provides a tertiary referral inpatient service and outpatient clinics, and services emergency inpatient care including paediatric intensive care unit admissions for patients diagnosed with diabetes aged under 19 years. The Department of Paediatric Endocrinology and Diabetes (DPED) teams consists of Paediatric endocrinologists, Diabetes Clinical Nurse Consultants, Dietitians, Social workers, and a Clinical Psychologist. In addition to the assessment and ongoing management for newly diagnosed patients and patients with existing diabetes, DPED offers stabilisation programs for newly diagnosed and existing patients with sub-optimal control, continuous blood glucose monitoring and insulin pump therapy initiation and review and a school diabetes education program.
About the Role
The Paediatric Diabetes Nurse Educator role is part of the multidisciplinary Department of Paediatric Endocrinology & Diabetes (DPED) established to support young people and families living with diabetes.
The Paediatric Diabetes Nurse Educator (DNE) practises independently and interdependently, assuming accountability and responsibility for his/her own actions. The Paediatric DNE will promote the delivery of a high standard of evidence-based nursing care to children and adolescents with diabetes within Monash Children’s Hospital (MCH) Clayton Campus. This includes the education of nursing and midwifery employees in the assessment, planning, implementation and evaluation of care in collaboration with children and adolescents and the Department of Paediatric Endocrinology & Diabetes (DPED) multidisciplinary team to achieve these goals and health outcomes.
The role will be responsible for:
- Deliver patient and family – centred care and education to children, adolescents and their families/carers diagnosed with diabetes.
- Responsible for providing evidence-based self-management education and care activities for children and young people and their families diagnosed with diabetes of any type
- Work in a multidisciplinary team to deliver a high standard of clinical case management, including patient assessment, the development of management plans and evidence-based diabetes education.
- Attend medical and joint nursing and allied health clinics as required
- Provide telephone advice and support for insulin adjustments/sick day management as per Monash Children’s Hospital (MCH) protocols and within scope of practice.
- Implement and participate in DPED clinics at Monash Children’s Hospital in collaboration with the multidisciplinary team
- Educational support pertaining to transitioning insulin modality from injections to pump therapy, including pump upgrades.
About You
Your attributes and desired experience:
- Post Graduate Certificate of Diabetes Education
- Australian Diabetes Educators Association (ADEA) Credentialed Diabetes Educator
- Significant experience in paediatric diabetes education
- Excellent communication and problem-solving skills
- Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia (NMBA) administered though the Australian Health Practitioner Regulation Agency (APHRA)
- Proficient in Hybrid Closed Loop Insulin Pump Therapy and Continuous Glucose Monitoring Systems commencement, patient education and review
- Current acute clinical diabetes experience in the hospital setting with the paediatric/adolescent population including those with Type 1 / 2 Diabetes, Neonatal Diabetes, Cystic Fibrosis Related Diabetes, Steroid Induced Diabetes
