About Aged and Rehabilitation

 

The Aged, Rehabilitation and Community Service brings together a range of medical specialities that deliver inpatient, ambulatory, consultative and community-based services across Monash Health. The service is pivotal in facilitating an optimal patient journey out of acute hospitals and ultimately out into the community, as well as facilitating hospital avoidance. It is key to Monash Health's strategic objective of delivering more care outside of the hospital.

 

About the Role 

 

Reporting to the Service Medical Director, the GP is a senior leader providing high-quality General Practitioner Services at Boollam Boollam, Monash Health's new Residential Aged Care Facility located at Kingston Centre in Heatherton.

 

What you need: 

 

  • Mandatory - Qualified as a medical practitioner with specialist qualifications and registration, or eligible for registration, as a specialist with the Medical Board of Australia in the appropriate discipline

  • Registration with the Australian Health Practitioner Regulation Agency (AHPRA)

  • Fellowship of the RACGP

  • Working with Children's Check (WWCC)

  • Australian Driver's License
