Our Journey
Monash at Home – Aged and Rehabilitation Care (ARC) is a fundamental part of the broader Aged and Rehabilitation Division. The team consists of Medical, Nursing, Allied Health, Pharmacy, Psychology and Adminstration staff. Monash at Home (ARC) provides an alternative to inpatient care, and providing safe and high quality home based services for individuals with complex medical, functional and cognitive issues, who require comprehensive, medically led multidisciplinary assessment and intervention within the comfort of their own home. Monash at Home ARC is a 40 bed services that covers the Monash Health catchment. The service operates out of two sites, Kingston Centre (Cheltenham) and Cranbourne, and staff are required to work flexibly across both sites to meet patient needs.
Patients within the service receive a daily visit from nursing. Medical, allied health, neuropsychology and pharmacy also provide face to face visits or Telehealth which are dependent on patient needs. Patients also have access to 24 hour telephone support.
The service also aims to provide short term, high quality, safe and effective patient centred care to patients within the primary Monash Health catchment. The service has 40 beds and operates out of Kingston Centre and at Cranbourne.
The Grade 2 Occupational Therapy role involves conducting face to face, and/or Telehealth Occupational Therapy assessments and intervention to patients who are admitted to the service. They are required to provide best practice patient care, achieve maximal operational effectiveness and support Monash Health's Strategic Guiding Principles.
We are looking for a dynamic Grade 2 Occupational Therapist to join our innovative team on a full time basis to support our patients to receive the best clinical care at home. Business hours are Monday-Fridays 0800 – 1630 hrs.
A Day in the Life
The Grade 2 Occupational Therapist will actively contribute to:
- Providing a high standard of client assessment, intervention and coordination of services, required aids and equipment to patients, to facilitate safe, effective and person-centred care
- The planning, development, quality and evaluation of evidence-based Occupational Therapy service across the continuum of care
- Participating in the development of a collaborative inter-professional team approach to achieve optimal patient care and outcomes
- Create strong partnerships within and outside Monash Health in collaboration with the Senior Clinician Occupational Therapist, Allied Health and Psychology Manager, Nurse Manager, and other team members
- Assuming responsibility for relevant administrative portfolios as directed by the Allied Health and Psychology Manager or the Senior Occupational Therapist
- Providing regular high quality clinicial supervision to Grade 1 Occupational Therapists and Allied Health Assistants as allocated
About you
- You are a dynamic Occupational Therapist with a genuine passion and interest in working with patients within a home environment, combined with proven experience and effective
- You have experience in working within a healthcare setting
- You have proven ability to work both independently and collaboratively and readily build strong working relationships with key stakeholders in the delivery of client centred care
- You have excellent communication and interpersonal skills
What you need
- Demonstrated recent experience as an Occupational Therapist with at least 4 years of clinical expoerience
- Current AHPRA registration
- Australian working rights
- Valid Driver's licence
