About the Role:
We are seeking a fixed-term Grade 2 Occupational Therapist to join our team at the Transition Care Program (Grade 3 considered if appropriate experience). This is a full time role, although part time applicants will be considered. The role will be based predominately at our Regis Dandenong North site, however, travel between Dandenong North, Kingston Centre and Cranbourne sites may be required. This fixed-term contract is until 30th June 2025.
A week in the life of the TCP OT is very diverse and autonomous including:
- A hands on clinical case load
- Supervision and case review of complex clients
- Supporting management in service delivery and re-design
- Providing clinical leadership and close collaboration with geriatricians and brokered care provider
- Leading Structured Clinical Incident Reviews in collaboration with Residential Aged Care bed-based provider and TCP Nursing Senior Clinician
About Us:
The Transition Care Program (TCP) is a major policy initiative between the State and Commonwealth Governments. TCP aims to minimise the number of older people experiencing inappropriate extended hospital lengths of stay, being prematurely admitted to residential care and/or prevent unnecessary presentations or admission to the Emergency Department.
The program provides short term support and active management for older people at the interface of the acute/subacute and aged care (i.e. community and residential) sector. The program is goal orientated to optimise the patient’s functional capacity and finalise and access their longer-term care arrangements or maintenance at home TCP can be provided as a bed based or as a home-based service.
By offering case management, low-intensity therapy and personal support, TCP allows older people more time and support in a non-hospital environment to complete their restorative processes, optimise their functional capacity and finalise their long term care arrangements.
About You:
The TCP OT will work collaboratively with the Allied Health and Nursing Senior Clinicians and the TCP Manager to provide clinical leadership to TCP across both the bed-based. The TCP OT will work closely with the local team to deliver quality services to clients, through strong clinical leadership and oversight, through ensuring that relevant policies and procedures are implemented to support best practice across the program.
To ensure your success in this role you will bring the following demonstrated skills, experience and knowledge:
- Demonstrates clinical skills and expertise in the area of community care and gerontology
- Ability to work autonomously and collaboratively with the multidisciplinary team to deliver safe consumer- focused care, providing specialist knowledge or depth of experience
- Provides holistic and comprehensive assessments and interventions
- A well-developed understanding of and ability to navigate complex service systems including (but not limited to) My Aged Care, State-wide Equipment Program and GEAT2GO
Mandatory requirements of the role:
- Current Victorian drivers licence
- Current and Satisfactory Aged Care Clearance (Police Check/Aged Care Worker Screen)
- NDIS workers check
- No current Aged Care Banning Orders in place (as per Register of Banning Orders)
- Registered with State-Wide Equipment Program
- Registration with GEAT2GO
