Our Journey
The Proactive Support Team within the Community Assessment and Response Team directorate offers a variety of programs that cover many aspects of health from screening right the way through to managing chronic conditions, covering the Monash Health Catchment including Greater Dandenong and Cardinia Casey. We focus on the assessment and delivery of client centred care. The team consists of Dietitians, Physiotherapists, Counsellors, Exercise Physiologists, Diabetes Nurse Educators, Care Coordinators, Psychologists, and Social Worker & Occupational Therapists and Clinical Nurse Consultants. The team is committed to working with communities, families and individuals who have poor health outcomes and to providing high quality, multidisciplinary programs which embrace client centred care, evidence based practice and the Social Model of Health.
We are looking for a dynamic Grade 2 Physiotherapist to join our team ongoing on a Part time basis to support our clients to access our community service.
This position is based in Doveton, but may be required to work at any of the Monash Health sites. The incumbent will be required to work in any clinical area at any or all campuses of Monash Health as and when required. Such a requirement may be a temporary arrangement, on an ad hoc basis, or a permanent change.
A Day in the Life
The Grade 2 Physiotherapist is responsible for working within the Proactive Support team to:

Providing a, independent and high standard of client assessment, treatment, management strategies and coordination of services to patients, to facilitate safe, effective, and person centred care often in the home and community setting.

Plan, develop, and evaluate evidence based Physiotherapy and services across the continuum of care.

Create strong partnerships within and outside Monash Health in collaboration with Senior Clinician Physiotherapists, Team Leaders, and other team members.

You will be responsible for providing clinical care to clients with Chronic Respiratory Disease, Chronic Heart Failure, and clients with complex psychosocial concerns.
About You:

You are a dynamic Physiotherapist with a genuine passion and interest in this unique opportunity, combined with proven experience and effective clinical assessment and intervention skills.

You have experience working with clients who have chronic conditions.

You can provide clinical care related to airway clearance, breathing strategies, energy conservation and muscle conditioning for clients with Chronic Respiratory Disease and Heart Failure.

With proven ability to work both independently and collaboratively, you readily build strong relationships with stakeholders in the delivery of client centred care.

You have excellent communication and interpersonal skills.

You are an advocate of Monash Health’s Strategic Guiding Principles.
What you need:

Demonstrated recent experience as a Physiotherapist with at least 5 years post graduate experience

Current AHPRA registration

Australian working rights
-
Valid Driver's licence
