About us:

We have an exciting opportunity for experienced Grade 2 Social Workers to join Clayton Adult Mental Health Program at Monash Health.

Adult Inpatient Unit in Clayton has 25 beds offering treatment to consumers. As the successful applicant, you are expected to provide specialist social work interventions and short term care coordination support to individuals who have presented to the Clayton bed based mental health service.

The role will undertake screening, assessment and access to community supports to reduce their psychosocial stressors impacting upon their mental state and impacting on their discharge. This role will also link very closely with the respective clinics in the Clayton catchment to endeavour to achieve seamless transition for clients and involve their families and carers throughout their recovery.

You will work within a multidisciplinary team and be part of a valued team of Social Workers providing support to consumers receiving direct care and on-going treatment (individual and group-based). All work is expected to be conducted in a way that upholds the Monash Health and Mental Health Program’s Vision, Purpose and Values and as a Monash Health employee, you will have the ability to maintain a high level of communication with patients, visitors and colleagues enabling you to deliver exceptional care.

 

What you bring: 

  • You are a qualified and experienced Social Worker with post graduate experience providing a strong focus on client centred care. 
  • Through your hard working and enthusiastic nature you have the ability to adapt to changing settings  
  • You are a dynamic Allied Health professional who is passionate about making a positive contribution to the health of your patients
  • You are highly motivated to perform at your best every day, demonstrating excellent communication and interpersonal skills 
  • Your dedication to safe best practice and quality care 
  • An unwavering commitment to delivering patient centred care 
  • Possess a positive attitude to your work, compassion and the ability to work collaboratively and effectively in a team 
  • Motivation to be accountable for and manage specific portfolios in our relentless pursuit of excellence 
  • Experience in a hospital or other health setting is essential. 

 

What you need:

To be considered for this important role you will ideally possess the following: 

  • An AASW accredited Bachelor degree or entry level Master’s Degree of Social Work that provides eligibility for membership of the Australian Association of Social Workers  
  • Good organisational skills and a positive, enthusiastic attitude towards work 
  • Experience in working with culturally and linguistically diverse communities 
  • Able to work autonomously 
  • A friendly, outgoing and reliable demeanour 
  • Well-developed clinical assessment and complex discharge planning skills  
  • A current Victorian drivers licence
