About Us

The Southern Melbourne Targeted Support for Children in Care, the Pathway To Good Health (PTGH), project is a new partnership between the Monash Health Developmental and Community Paediatric Unit and the Victorian Department of Health.  The aim of the PTGH Service is to provide consistent and high-quality models of care to children entering statutory care settings.  Southern Melbourne PTGH will be one of seventeen services in DFFH areas across Victoria. 

 

 

About the Role

Step into a rewarding opportunity to make a meaningful impact as an Allied Health Health Navigator (Grade 2 – Speech Pathologist, Occupational Therapist, or Physiotherapist). This part-time role (0.60 EFT / 45.6 hours per fortnight) is offered on a 12-month fixed-term basis, with a required workday on Monday and flexibility across other shifts.

 

In this dynamic position, you will play a key role in supporting children entering the PTGH program within the local DFFH Area by gathering and screening essential health information.

 

You will conduct early health and social history information gathering and, where appropriate, undertake brief, within-scope functional screening to support triage (not clinical assessment). Working collaboratively with the PTGH Clinical Coordinator, you will develop comprehensive health summaries and recommendations to help ensure each child’s health and wellbeing needs are effectively supported.

 

 

About You

You are a motivated and adaptable Allied Health professional who is passionate about supporting children and their wellbeing. You bring strong clinical insight, excellent communication skills, and a collaborative approach to your work.

 

To thrive in this role, you will have:

  • A recognised tertiary qualification in an Allied Health profession
  • Current registration or accreditation with the appropriate professional body (AHPRA or equivalent)
  • Confidence in clinical documentation and record keeping, with experience using electronic health record systems
  • A current driver’s licence and willingness to travel between sites as required
  • Highly developed verbal and written communication skills, with the ability to build strong relationships
  • A commitment to ongoing professional development and continuous learning
  • The ability to work autonomously, manage your time effectively, and adapt within a dynamic, evolving environment
