About the Role:

We have a job share opportunity to work as a Heart Failure Clinical Nurse Consultant in Ambulatory Services at Victorian Heart Hospital.

  • Two part time fixed-term positions available:

             - 0.5 EFT , from Nov 2025 - June 2026

             - 0.5 EFT, from Nov 2025- Jan 2027

  • Opportunity to flex to full time for annual leave cover

  • Monday - Friday from 0800-1630 (no weekends)

 

The VHH Heart Failure Clinical Nurse Consultant (CNC) position is responsible for the identification and recruitment of eligible patients to the program, including triage of inpatient and outpatient referrals (accordingly to established guidelines). The position coordinates bookings for the Heart Failure clinic.

The VHH Heart Failure CNC is also responsible for referring patients to HF specific rehabilitation programs. The role is expected to demonstrate skills in holistic patient care, considering patient comorbidities, emotional health & adjustment to new/chronic diagnoses, social network, cultural beliefs & needs.

The role provides heart failure specific education to general Registered Nurses across VHH as well as maintaining personal professional development with self-directed and conference-based learning.

The Victorian Heart Hospital is Australia’s first dedicated heart hospital which includes a full range of ambulatory and inpatient cardiac care, including cardiac theatres, cardiac catheterisation laboratories, emergency, intensive care, and ambulatory services such as cardiac CT, echocardiography and specialist consultation. It features a unique, patient centred model of care. The Victorian Heart Hospital plays a key role in advancing research and teaching and is the home to the Victorian Heart Institute, a brand new, health and medical research organisation at Monash University.

Heart Failure Clinical Nurse Consultant, Ambulatory Services Victorian Heart Hospital

The VHH Heart Failure program is a nurse-led cardiologist supported program which delivers comprehensive, guideline indicated therapy to patients with newly diagnosed Heart Failure with reduced Ejection Fraction (HFrEF), paying particular attention to patient education (heart failure self-management) and supported transition from hospital to the community. The program also caters for patients with longstanding cardiomyopathy with recent hospitalisation/decompensation and those requiring treatment stabilisation or heart failure device referral.

  •  Post-graduate qualification in area of speciality e.g., Critical Care, Cardiovascular Nursing or Cardiac Nursing
