About Us
Monash Health is Victoria’s largest public health service, and we are looking for an experienced and motivated Nurse Manager to lead our Monash at Home – Aged and Rehabilitation Care (ARC) nursing team. This unique leadership role supports the delivery of safe, high-quality, home-based care as an alternative to hospital admission.
About the Role
As the Nurse Manager, you will lead and manage the Monash at Home nursing team comprising of Registered Nurses working on the road and Clinical Care Coordinators who are office based across the Kingston and Cranbourne sites, working in partnership with the Allied Health & Psychology Manager and Medical Lead as part of the Monash at Home governance team.
In this leadership role, you will be responsible for driving quality and safety improvements to ensure patient-centred care in the home environment, while overseeing integrated operational management including workforce, finance, quality, and safety.
You will also participate in a rostered on-call system to provide 24/7 telephone support to patients and contribute to the development of innovative models of care designed to support earlier hospital discharges and reduce the need for subacute hospital admissions.
This is an ongoing Full Time, Monday to Friday opportunity.
What you Bring:
To be successful in this role, you will hold current AHPRA registration as a Registered Nurse and bring proven leadership experience in aged, subacute, rehabilitation, or home-based care. You will demonstrate strong skills in clinical governance, workforce planning, and operational management, along with the ability to build effective relationships across multidisciplinary teams. Above all, you will have a genuine passion for supporting patient dignity, independence, and person-centred care.
