About the Role
Join our team as an Occupational Therapist, Grade 2, on a full-time fixed-term basis (maternity leave cover) until June 2026, to work with the Aged and Community Care Program. Based in the City of Greater Dandenong (Springvale/Dandenong) and City of Casey (Cranbourne), you may also be required to travel to other Monash Health sites.If you are a Grade 1 looking for the next step in your career you are strongly encouraged to apply!
You will work collaboratively within a multidisciplinary team to support clients to promote independence and to remain living at home and within the community for as long as possible. The role requires provision of evidence-informed assessment, intervention and discharge planning to clients within the Aged and Community Care Program. You will work with clients and/or carers to increase their engagement in meaningful occupation and support healthy living options.
Services can take place in a client’s home, in the community and/or by telehealth.You will also be responsible for working within the Aged and Community Care program to provide best practice client care by, using specialist skills and knowledge to conduct client assessments and interventions independently. Incorporating remedial, compensatory, educational and preventative strategies to achieve optimal evidence-based consumer care.

About Us
The Aged and Community Care (ACC) Program provides an interdisciplinary service to support older people in the community. The program aims to keep the client independent within their community, proactively promoting health and independence, and self-management whilst assisting with timely interventions to prevent deterioration of their condition, and admission to hospital or residential care.
About You

- Bachelor of Occupational Therapy or relevant equivalent degreeCurrent AHPRA registration
