About the Role

Join our team as an Occupational Therapist - Grade 2, on a full time, fixed term basis until June 2026, in the Primary Care and Support Team based across multiple Monash Health locations. 

 

As a Grade 2 Occupational Therapist, you will use your specialist knowledge and skills to conduct client assessment and intervention independently, incorporating remedial, compensatory, educational and preventative strategies to achieve optimal evidence-based consumer care and to support consumer health and wellbeing through occupation and consideration of the person and their environment. You will also assist with the development of strong, evidence based practice within Primary Care Support Program and as well as the broader Community Assessment Response Team (CART).

About Us

Primary Care Support Team, as part of the Community Assessment and Response Directorate offers a variety of programs that assist individuals to manage chronic health conditions, focusing on the assessment and delivery of client focused care. The team consists of Allied Health Assistants, Care Coordinators, Community Development Workers, Counsellors, Diabetes Nurse Educators, Dietitians, Exercise Physiologists, Leisure & Lifestyle Assistants, Psychologists, Occupational Therapists, Physiotherapists, Recreational Therapists, Social Workers, and Podiatrists. The team is committed to working with communities, families and individuals who have poor health outcomes and to providing high quality, multidisciplinary programs which embrace client centred care, evidence based practice and the Social Model of Health. 

 

About You

To ensure your success in this role you will bring the following demonstrated skills, experience and knowledge:

  • Dedication to safe best practice & quality care 

  • Committed to delivering patient-centred care & patient advocacy 

  • Enthusiastic, compassionate & able to work collaboratively 

  • Bachelor of Occupational Therapy or relevant equivalent degree 

  • Current AHPRA registration 

  • Current Driver’s License
