About the Role
Monash Health is reimagining the way care is led and delivered. With a refreshed organisational structure, we’re building a leadership team designed for the future. The Operations Manager – Care in the Home is a key leadership role, responsible for a portfolio of diverse health services designed to enhance patient outcomes and promote independence by integrating the following key areas:
- Hospital in the Home
- Community Assessment and Response Team
- Monash at Home
- Community Rehabilitation
- Residential InReach
Reporting to the Operations Director – Community and Care in the Home, if appointed, you will take accountability for the service delivery of holistic, patient-centred care, empowering clients to manage their health within their communities.
Responsibilities include:
- Driving high-quality patient outcomes and service performance
- Leading and developing a skilled and engaged workforce
- Managing financial performance and resource allocation
- Promoting innovation and contemporary models of care
- Ensuring compliance with safety, quality, and governance standards
- Building strong, enduring relationships with internal and external stakeholders
This role plays a pivotal part in shaping the behavioural framework and workplace culture, empowering the workforce to embrace innovation, adopt contemporary care models, and explore new service opportunities.
This is a site-based role operating Monday to Friday during business hours. Some travel between Monash Health sites will be required and a current Driver’s Licence is a mandatory requirement.
About You
With a qualification in nursing, midwifery, allied health, medicine or a related field, and registration with the Australian Health Practitioner Registration Authority (AHPRA), or eligibility for registration with the peak professional body for non AHPRA Registered professions, you’re an experienced passionate health leader, with demonstrated success in leading teams in complex environments.
You demonstrate reflection and insight and have a collaborative mindset. Energetic, enthusiastic and thoughtful, you’re seen as an effective leader people seek to follow. Guided by and embracing Monash Health’s values of integrity, compassion, accountability, respect and excellence, you relish the challenge of playing a key role in delivering meaningful change.

You’ll lead a diverse portfolio that includes a range of specialist services impacting both community members in greatest need of care as well as helping prevent avoidable hospital presentations and enabling patients to receive either acute or subacute care in their usual residential settings.
