Join Monash Health as an Anticoagulation Stewardship Pharmacist and help shape the future of patient care.  This is a part time (4 days week), fixed term opportunity covering parental leave, until December 2026.

  

About the Role  

 

As Anticoagulation Stewardship Pharmacist, you will be an integral part of the multidisciplinary team at Monash Health, working alongside doctors, nurses, and allied health professionals to deliver high-quality care across inpatient, ambulatory and home-based settings.  

 

You will be joining Victoria’s largest hospital pharmacy department, where you will gain exposure to a wide range of clinical experiences. Our dedicated Education Team will support your growth through tailored training programs designed to enhance your clinical and leadership skills. 

  

A Day in the Life 

 
Working closely with the Pharmacy Team, you will be recognised as a specialist in anticoagulation. You will deliver high quality clinical services to patients and carers whilst continuing to build on your existing knowledge and experience. You will also be provide training and development to the pharmacy team and other disciplines across the organisation. 

 

You will lead quality improvement and research initiatives and will have the opportunity to engage with Pharmacy professional groups outside Monash Health. Applicants seeking Grade 4 will need to submit a portfolio of evidence demonstrating relevant experience and impact.

About You 

 

You bring extensive hospital pharmacy experience and a strong interest in anticoagulation stewardship and haematology services. Your professionalism shines through in everything you do – you lead by example and consistently set high standards for others to follow. Your positive attitude fosters a collaborative and supportive work environment, making you a pleasure to work with.  

 

Post graduate qualifications such as a Masters of Clinical Pharmacy or equivalent, will be highly regarded and reflect your commitment to ongoing professional development. 

  

You can also demonstrate: 

  

  • Leadership experience 

  • Commitment to research, education and professional contribution 

  • Current AHPRA registration as a Pharmacist.
