About the Role

Join our team as a Cardiology pharmacist on a full time, parental leave cover until 2nd July 2027, to work at the Victorian Heart Hospital in Clayton.  In this role, you will be guided under the leadership of the Pharmacist Team Leader for Cardiology, Cardiac Surgery and Heart Failure Clinics, working with the broader pharmacy and multidisciplinary teams.

 

Key responsibilities include:

  • Delivering best-practice clinical pharmacy services in cardiology and cardiac surgery
  • Providing specialist medication management expertise to multidisciplinary teams
  • Supporting patient flow through medication reconciliation and pharmacist-led discharge planning
  • Participating in the education, supervision and mentoring of pharmacy staff and students
  • Contributing to research, audit and quality improvement initiatives.

 

This role includes weekends, public holidays, late shifts and on-call, and may require support across other pharmacy services as sites as needed.

  • Demonstrated experience in hospital pharmacy and cardiology services
  • Strong clinical judgement and patient-centred approach to care
  • Excellent communication skills and ability to work collaboratively in multidisciplinary teams
  • Commitment to education, quality improvement and professional development.

 

Mandatory requirements:

  • Current registration with AHPRA

About the Department/ Program

The Monash Health Pharmacy Program is one of the largest in Australia, comprising over 550 pharmacy professionals working across acute, specialist and community settings. Our Cardiology Pharmacy team is primarily based at the Victorian Heart Hospital, the only cardiac-specific hospital in the southern hemisphere and the only hospital with a cardiac emergency in Australia.

The cardiology pharmacy team works collaboratively within multidisciplinary environments including cardiology, cardiac surgery and heart failure services, supporting safe, evidence-based care while contributing to education, research, and quality improvement initiatives.
