About the Department / Program

Monash Health Pharmacy is a diverse and dynamic team of over 500 professionals. We work across seven hospitals and community settings, delivering best-practice pharmacy services and supporting career development through structured rotations, mentoring, and leadership opportunities.

 

About the Role

Join our team as a Quality Use of Medicines Pharmacist on a full time, ongoing basis. This role works across sites.

 

Working alongside the Quality Use of Medicines and Adverse Reactions Team Leader and broader multidisciplinary team, you’ll deliver expert Medication Safety services while contributing to education, quality improvement and ongoing patient safety. Delivering high quality, safe and innovative care and best practice pharmacy in the area of Medication Safety

 

Key responsibilities include:

  • Active participation in the preparation for National Safety and Quality Health Service Standards accreditation, with a particular focus on Standards 3 and 4.
  • Representing the Pharmacy and/or provide medication safety expertise on pharmacy committees and interprofessional organisational committees.
  • Supporting the activities of the Monash Health Medication Safety and Therapeutics Committee including medication incident management, audits, quality reporting, risk management, document management and improvement strategies.
  • Working within the multidisciplinary team to provide expertise in medication management and promoting the safe and effective use of medicines.
  • Assisting in the review of incidents, generation and dissemination of monthly quality reports, relevant quality improvement summaries and/or plans and risk registers.
  • Providing communication to clinical staff regarding medication safety strategies and projects.

 

About You

We’re looking for a pharmacist who is passionate about Medication Safety, enjoys working as part of a multidisciplinary team and is committed to improving patient outcomes.

 

You will bring:

  • Hospital pharmacy experience, ideally with Medication Safety experience
  • Demonstrated ability to work effectively in fast-paced, complex clinical environments
  • Strong communication, stakeholder engagement and collaboration skills
  • Experience in clinical governance, quality improvement and medication safety
  • Excellent organisational, decision-making and problem-solving skills

 

Mandatory requirements of the role

  • Registered with the Australian Health Practitioners Agency (AHPRA) Pharmacy Board 
  • Relevant clinical pharmacy experience
  • Relevant post graduate qualifications (completed or in progress)
  • Clinical Supervision training (desirable)
