About the Role:
We are seeking a motivated Grade 2 Physiotherapist to join our Primary Care Support Team as part of the Community Assessment and Response Team, on a full-time ongoing basis.
In this role, you will deliver evidence-based physiotherapy assessments and interventions to clients with a wide range of clinical presentations, often with multiple co-morbidities. You will also have the opportunity to facilitate group programs.
As part of the broader Monash Health Physiotherapy service, you’ll benefit from access to professional development, supervision, and a breadth of both generalist and specialised clinical experiences. About Us:
Primary Care Support Team, as part of the Community Assessment and Response Team (CART) offers a variety of programs that assist individuals to manage chronic health conditions, focusing on the assessment and delivery of client focused care. This is a new service in Monash Health and an exciting opportunity to be part of our growth.
The CART team consists of Allied Health Assistants, Care Coordinators, Community Development Workers, Counsellors, Diabetes Nurse Educators, Dietitians, Exercise Physiologists, Psychologists, Occupational Therapists, Physiotherapists, Social Workers, Recreational therapists, Lifestyle and Leisure Assistants, Nurses and Podiatrists.
We are committed to working with communities, families and individuals providing high quality, multidisciplinary programs which embrace client centred care, evidence based practice and the Social Model of Health. We are currently building on an Outreach service that is an exciting new space to work in.

Mandatory Requirements:
- Bachelor of Physiotherapy (or equivalent qualification)
- Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) – Physiotherapy Board of Australia (practicing, no restrictions)
- Current Driver’s License
