About the Opportunity

We’re seeking an enthusiastic and experienced Grade 2 Physiotherapist to join the CART Intensive Support Team at Monash Health on a part-time, ongoing basis (0.42 EFT / 32 hours per fortnight). The role is primarily based at Berwick Community Health and involves working across multiple sites. 

 

This is a unique opportunity to work in a short-term, high-impact service supporting patients soon after admission to acute or subacute care, bridging the critical gap while they await ongoing community physiotherapy.

 

You’ll step into a role where your clinical skills are used at pace and purpose—helping people regain function, confidence, and independence at a pivotal point in their recovery journey.

  • Work across multiple Monash Health sites and undertake home visits

About Us

CART delivers a tiered, person-centred model of care that targets support where it is most needed—helping people remain safely in the community and avoid unnecessary hospital presentations.

 

Working in partnership with acute and primary care services, CART supports smooth transitions across the healthcare system and provides short-term care coordination for individuals with chronic conditions and frequent hospital use, with a strong focus on self-management and continuity of care.

 

 

About You

You are a confident, experienced Physiotherapist who enjoys variety, autonomy, and meaningful client interaction.

 

You bring strong clinical reasoning and assessment skills, along with the ability to engage, educate, and empower clients and carers to support safe recovery at home.

 

You thrive in environments where you can work both independently and as part of a collaborative team, and you are motivated by improving outcomes for people with complex needs.

 

You will bring:

What you need

  • Minimum 5 years’ post graduate Physiotherapy experience
  • Bachelor of Physiotherapy (or equivalent)
  • Current AHPRA registration (no restrictions)
  • Current Driver’s Licence
  • Postgraduate study (desirable)
  • Australian Physiotherapy Association membership (desirable)
