Part-time ongoing | 32 hours per fortnight (0.42 EFT) | CART Intensive Support Team
Based primarily at Berwick Community Health and working across multiple sites, this is a unique opportunity to support patients at a critical stage of their recovery journey.
Imagine being the clinician who helps someone return home with confidence after a hospital stay.
As part of the CART Intensive Support Team, you'll provide timely physiotherapy intervention to patients following admission to acute or subacute care, helping bridge the gap while they await ongoing community physiotherapy services. You'll use your clinical skills where they matter most - supporting people to regain function, maintain independence, and safely navigate their recovery at home.
This is a role where your expertise has a direct and visible impact. Working across community and home-based settings, you'll support patients during a short but highly influential phase of care, helping improve outcomes and prevent avoidable hospital presentations.

You will:
- Work across multiple Monash Health sites and undertake home visits
- Support clients during a short but highly influential phase of care
- Collaborate closely within a skilled multidisciplinary team
- Play a key role in preventing avoidable Emergency Department and acute presentations
- Help patients and families build confidence to self-manage at home
About Us
The Community Access and Response Team (CART) delivers a tiered, person-centred model of care that targets support where it is most needed - helping people remain safely in the community and avoid unnecessary hospital presentations.
Working in partnership with acute and primary care services, CART supports smooth transitions across the healthcare system and provides short-term care coordination for individuals with chronic conditions and frequent hospital use, with a strong focus on self-management and continuity of care.
About You
You are a confident and experienced Physiotherapist who enjoys variety, autonomy, and meaningful client interaction.
You bring strong clinical reasoning and assessment skills, along with the ability to engage, educate, and empower clients and carers to support safe recovery at home.
You thrive in environments where you can work both independently and as part of a collaborative team, and you're motivated by improving outcomes for people with complex needs.
You will bring:

What You Need
- Minimum 5 years' postgraduate Physiotherapy experience
- Bachelor of Physiotherapy (or equivalent)
- Current AHPRA registration (no restrictions)
- Current Driver's Licence
Desirable:
- Postgraduate study
- Australian Physiotherapy Association membership
