About the Role:

Join our team as a Physiotherapist Grade 2 on a fulltime ongoing basis within the Transition Care Program (part-time applicants will also be considered). Based predominantly at our Regis Dandenong North site, however, travel between Dandenong North, Kingston Centre and Cranbourne sites may be required.

In this exciting role, you’ll be at the forefront of delivering high-quality clinical care and leadership within our Aged Care Services Program. You’ll manage a dedicated caseload, while also supporting the broader needs of the Aged Care Services team.

Key Responsibilities:

  • Provide evidence-informed physiotherapy assessments, care planning, interventions, and discharge planning to clients
  • Offer physiotherapy services within clients homes, residential care settings, and group programs
  • Be an active member of the physiotherapy and multidisciplinary teams, contributing to best practice clinical care. Share your expertise by mentoring students and colleagues, and engage in research, quality projects, and administrative duties

About Us:

The Transition Care Program (TCP) is a major policy initiative between the State and Commonwealth Governments.

 

TCP aims to minimise the number of older people experiencing inappropriate extended hospital lengths of stay, being prematurely admitted to residential care and/or prevent unnecessary presentations or admission to the Emergency Department. The program provides short term support and active management for older people at the interface of the acute/subacute and aged care (i.e. community and residential) sector. The program is goal orientated to optimise the patient’s functional capacity and finalise and access their longer-term care arrangements or maintenance at home.

 

TCP can be provided as a bed based or as a home-based service. By offering case management, low-intensity therapy and personal support, TCP allows older people more time and support in a non-hospital environment to complete their restorative processes, optimise their functional capacity and finalise their long term care arrangements. 

 

 

About You:

The TCP Physiotherapist will work collaboratively with the Allied Health and Nursing Senior Clinicians and the TCP Manager and the local team to deliver quality services to clients, through strong clinical leadership and oversight, through ensuring that relevant policies and procedures are implemented to support best practice across the program.

 

To ensure your success in this role you will bring the following demonstrated skills, experience and knowledge:

  • Demonstrates clinical skills and expertise in the area of community care and gerontology
  • Ability to work autonomously and collaboratively with the multidisciplinary team to deliver safe consumer- focused care, providing specialist knowledge or depth of experience
  • Provides holistic and comprehensive assessments and interventions
  • Bachelor of Applied Science (Physiotherapy) or equivalent
  • Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) 
  • NDIS Workers Check
  • Current Victorian drivers licence
