About the Role:
We have an exciting opportunity for a Grade 3 Physiotherapist to join the Primary Care Support Team, as part of the Community Assessment and Response Team on a part-time (0.4 EFT / 30.4 hours per fortnight), fixed-term basis until August 2026.
In this role you will provide evidence based physiotherapy assessments and interventions to clients with a wide range of clinical presentations, often presenting with multiple co-morbidities. This role will also be involved in running groups. Being part of the wider Monash Health Physiotherapy service, with access to all the benefits that offer: professional development, supervision, and broad and specialised clinical experience. About Us:
Primary Care Support Team, as part of the Community Assessment and Response Team (CART) offers a variety of programs that assist individuals to manage chronic health conditions, focusing on the assessment and delivery of client focused care. This is a new service in Monash Health and an exciting opportunity to be part of our growth. The CART team consists of Allied Health Assistants, Care Coordinators, Community Development Workers, Psychologists, Nurses , Dietitians, Exercise Physiologists, Occupational Therapists, Physiotherapists, Social Workers, and Podiatrists, Recreational Therapists, Leisure and Lifestyle Assistants. We are committed to working with communities, families and individuals providing high quality, multidisciplinary programs which embrace client centred care, evidence based practice and the Social Model of Health. We are looking forward to welcoming Grade 3 Physiotherapists to be part of our growth.

Mandatory Requirements:
- Bachelor of Applied Science (Physiotherapy) or equivalent
- Current registration with the Australian Health Practitioner Regulation Agency (AHPRA)
- At least 7 years clincal experience
