About the Program
The Community Assessment and Response Team (CART) at Monash Health is a multidisciplinary program designed to provide timely, community-based care for individuals with complex or chronic health conditions. Operating within the Monash Health Community Program, CART aims to reduce unnecessary hospital admissions and support clients in managing their health within their own homes and communities.
CART provides care through three main streams:

Intensive Management & Support: Comprehensive care coordination and outreach for clients with complex needs.

Proactive Support & Management: Early intervention and self-management support for chronic conditions.

Primary Care & Support: Short-term, goal-focused allied health services to maintain client independence.
We are seeking a experienced Community Health Nurse - Community Role based at Dandenong
This full-time, temporary role offers 1 EFT / 80 hours per fortnight within the CART Proactive Support and Management Team.
About the Role
This role provides care coordination to clients with chronic disease and / or complex psychosocial issues. The Care Coordinator works with the client and their family to improve their health and wellbeing, reduce avoidable presentation and admission to Monash Health hospitals and to engage with supports in the community.
Key Responsibilities:

Ensure the delivery of a high standard of evidence-based nurisng care to consumers

Conduct comprehensive assessment, planning, implementation, and evaluation of care in collaboration with the consumer and the multidisciplinary team to achieve goals and health outcomes
- Establish and maintain effective communication with all other health professionals and support employees, consumers, families, the community and external care providers

Support service KPIs, team meetings, and waitlist management.
About You
You are a highly effective communicator with strong, well-developed analytical and problem-solving skills. To ensure your success in this role you will also bring the following demonstrated skills, experience and knowledge:

Genuine passion and interest in this unique opportunity, combined with proven experience and effective clinical assessment and intervention skills

Experience working with clients who have chronic conditions

Proven ability to work both independently and collaboratively, you readily build strong relationships with stakeholders in the delivery of client centred care
- Sound understanding of the social determinants of health

Excellent communication and interpersonal skills
Mandatory requirements of the role:
-
Demonstrated recent experience as a Registered Nurse with at least 5 years post graduate experience
- Previous community Nursing experience desirable but not essential
- Sound understanding of community and outreach work
- Demonstrated ability to coordinate client care

Current AHPRA registration
-
Valid Driver's licence
- Current polic check and WWCC
- Proof of current immunisation status
