About Us:  

 

Community Health provides care to clients with chronic disease and/or complex psychosocial issues. The Community Health Community Clinical Nurse will work with the client and the family to improve their overall health and wellbeing and reduce avoidable presentations and admissions to acute Monash Health services and reduce length of stay when admitted.  

 

The focus of this role will be providing education, comprehensive assessment and coordination of care for clients who have Chronic Respiratory Disease. The aim is to ensure that the clients and families have good community supports and connections. The program utilises an assertive outreach model and all positions require home visits as appropriate to clients residing in the Monash Health catchment.  

 

About You: 

You are a dynamic Registered Nurse with a genuine passion and interest in this unique opportunity. You will mainly Monday to Friday with some coverage required on weekends and public holidays. As you will work across various Monash Health sites, and home visiting a valid driver’s licence is essential.  

 

What You need: 

Essential: 
 

  • Minimum 5 years post graduate experience in acute nursing, with current continuum of practice 

  • Demonstrated knowledge and experience of working in chronic disease management, chronic respiratory disease and experience using self-management approaches.  

  • Current AHPRA registration Valid Driver's licence Current Working with Children’s Check Current Police Check 

    Desirable:  

  • Post-graduate qualifications 

  • Demonstrated skills in assessment, care planning, service provision and the ability to function both independently and within an interdisciplinary team 

  • Sound understanding of community and outreach work 

  • A demonstrated knowledge of Chronic Heart Failure is desirable
