About the Role
Join our team as a CART Response Nurse on a full time, fixed-term or Ongoing basis to work at multiple Monash Health sites. CART Intensive Support / Response Team is seeking a motivated experienced nurse with excellent communication skills to join the team. The CART Response Nurse will prioritise Post Acute Care (PAC) and CART referrals to facilitate timely supported discharge to home. This position performs a key role in decreasing length of stay in acute settings by identifying clients who are eligible for Health Independence Program (HIP) services and assisting with streamlined referral from the acute setting to community care. This role is integral to bridging the gap between acute to Community services. Essential to the role is experience in general medical and surgical nursing as well as an understanding of community services. The role will require the successful applicant to work across the multiple Monash Health sites, including Casey Hospital, Dandenong Hospital and Monash Medical Centre.

What you need
Essential:

Demonstrated recent experience as a Registered Nurse

Current AHPRA registration

Valid Driver's licence
Desirable:

Previous Community Nursing experience

Recent acute experience
-
Post-graduate qualifications in critical care or similar
