About the Role:

Full-Time fixed – term opportunity ( December 2025 – March 2026)
In this position, you will provide high-quality nursing assessments and coordinated care for patients accessing the Disability Health Access Service (DHAS) at the Centre for Developmental Disability Health (CDDH). You will accept and screen referrals, complete telephone-based nursing assessments, and liaise with medical treatment decision makers, GPs, diagnostic imaging, theatre, medical, and anaesthetic teams.
You’ll deliver on-ground nursing support during DHAS appointments, complete follow-up nursing reviews, and escalate clinical concerns to the Head of CDDH as required. The role also contributes to quality improvement activities, multidisciplinary collaboration, and data collection supporting CDDH workforce development projects.
About Us:
The Centre for Developmental Disability Health (CDDH) is dedicated to improving health outcomes for people with intellectual and developmental disabilities. We support healthcare services through clinical work, education, and research.
The Disability Health Access Service (DHAS) provides targeted procedural sedation to enable people with intellectual and developmental disabilities to access essential preventative healthcare.
While primarily based in Doveton, this role includes travel to other Monash Health sites as part of clinical service delivery.
About You:
You are a dedicated and collaborative nurse, committed to delivering person-centred, evidence-based care. You bring strong assessment skills, clinical judgement, and the ability to work effectively within a multidisciplinary team.
You will have:

Registration with the Nursing and Midwifery Board of Australia via AHPRA
-
Experience working within a multidisciplinary team
-
Knowledge of tertiary healthcare systems
-
A valid Victorian Driver Licence
Highly desirable:
-
Experience working with adults with disabilities and their supports
-
Experience in community-based healthcare
-
Knowledge of the National Disability Insurance Scheme (NDIS)
-
Qualification as a nurse immuniser
