About the Role:

  • Join our team as a Community Health Nurse on a permanent part-time (0.8 EFT) basis to work with the Aged and Community Care Program

  • This position will work across multiple sites across Greater Dandenong Kingston and Casey Cardinia. 

You will work collaboratively within a multidisciplinary team to support clients to promote independence and to remain living at home and within the community for as long as possible.

Step into a rewarding day in community health where you’ll use your clinical expertise in gerontology to deliver holistic nursing and interdisciplinary assessments, personalised care planning, wound and pressure injury management, continence support, and meaningful education for clients and families—all in the comfort of their homes. 

  • You’ll build strong community connections, provide specialist service linkages, and contribute to the ongoing growth and innovation of our Aged and Community Care services. Services can take place in a client’s home, in the community and/or by telehealth. 

  • You will also be responsible for working within the Aged and Community Care program to provide best practice client care by, using specialist skills and knowledge to conduct client assessments and interventions independently.

  • Incorporating remedial, compensatory, educational and preventative strategies to achieve optimal evidence-based consumer care.  

 

About Us:

The Aged and Community Care (ACC) Program provides an interdisciplinary service to support older people in the community. The program aims to keep the client independent within their community, proactively promoting health and independence, and self-management whilst assisting with timely interventions to prevent deterioration of their condition, and admission to hospital or residential care.  

 

 About You:

To thrive in this role, you will bring the following:  

  • A strong passion to provide safe best practice nursing    
  • Commitment to patient-centred care delivery and advocacy   
  • Enthusiasm, compassion and ability to work collaboratively within a multidisciplinary team  
  • Bachelor of Nursing or relevant equivalent degree. 
  • Current Nursing Registration via the Australian Health Practitioner Regulation Agency (AHPRA), and consent to ongoing registration checks via Monash Health’s electronic credentialing system 
  • Current Victorian Driver’s License  
  • Current and Satisfactory Aged Care Clearance (Police Check/Aged Care Worker Screen)  
  • No current Aged Care Banning Orders in place (as per Register of Banning Orders) or NDIS exclusion order in place 
  • Experience in community nursing setting and/or gerontology
