About The Role  

We are looking for talented Registered Nurses to assist in the coordination of the Complex Care clinics. There are several clinics, including Chronic Heart Failure, Chronic Respiratory Disease, Chronic Disease Management and Supportive Chronic Conditions Clinic. These clinics are located at Dandenong Hospital, Monash Medical Centre, Doveton and Berwick.   

The role manages the client referral, appointments and care of the client within the clinics, communicates with the appropriate health professionals and services and follows up the plan of care. This role also involves attending to the nursing tasks that pertain to client care from the clinic, such as follow up of investigations and test results.   

The role will require the successful applicant to work across the multiple Monash Health sites, including Dandenong Hospital, Doveton and Berwick Community Health Centre.

The role may include undertaking additional duties in other program areas, as operationally required.

What you need

Essential:  

  • Demonstrated recent experience as a Registered Nurse  

  • Current AHPRA registration  

  • Australian working rights  

  • Valid Driver's licence  

Desirable:  

  • Previous Community Nursing experience  

  • Recent acute experience  

  • Post-graduate qualifications in critical care or similar
