The Clayton HOPE (Hospital Outreach Post-suicidal Engagement) program is a Monash Health mental health initiative designed to provide intensive support after a suicidal crisis or self-harm event, reducing the risk of repeat attempts.

 

This Victoria-wide program, originating at Monash Health in Clayton, offers up to 12 weeks of support through a multidisciplinary team, including:

  • Psychosocial support workers
  • Mental health clinicians
  • Peer workers with lived experience


Services include:

  • Developing personalised safety plans
  • Psychological support to build coping skills
  • Connecting individuals and families with community resources


Our model of care includes five stages:

  1. Early engagement and empathetic support (within 24 hours)
  2. Assessment of psychosocial needs and suicidal risk (within 72 hours)
  3. Construction of a personal safety plan (within 7 days)
  4. Implementation and risk management (weeks 2–12)
  5. Discharge and handover to ongoing supports (12 weeks)

 

What makes HOPE unique? The first contact is with a Peer Support Worker—someone with lived experience who offers empathetic, informed care.

 

About the Role

As a Senior Peer Worker in the HOPE team, you will:

  • Mentor and support Level Two Peer Workers
  • Provide reflective supervision to peers outside your team
  • Contribute consumer perspectives in psychosocial reviews and team meetings
  • Support non-peer workers with consumer insights
  • Build and manage a caseload of consumers
  • Deliver one-to-one and group support
  • Co-design and co-facilitate consumer groups
  • Work collaboratively within a multidisciplinary team

 

About You

This is a designated consumer lived experience role. You will:

  • Have a declared personal experience of living with mental illness and using mental health services
  • Bring at least one year of professional experience as a Level Two Consumer Peer Worker
  • Demonstrate knowledge of peer work principles and Intentional Peer Support (IPS)
  • Be passionate about inclusion, diversity, and person-centred care
  • Possess excellent communication and interpersonal skills
  • Be committed to supporting others and growing as a lived experience health professional
