About the Role

Join our team as a Senior Social Worker on a full time, ongoing basis, working within the Atherton Road Hub in Oakleigh. 

 

This specialised Social Work position at Monash Health’s Atherton Road Centre plays a pivotal role in supporting consumers, families, and carers through their mental health journey. With a strong focus on holistic, person-centred care, the role aims to ensure a seamless transition for clients moving into the Centre for Case Management, while actively involving and supporting their families and carers.

 

In this role, you will provide targeted psychosocial interventions and family-focused support, including psychoeducation and community linkage. While the position does not carry direct case management responsibilities, you will contribute expert, discipline-specific input to the broader multidisciplinary team, offering secondary consultation and mentoring support where needed.

 

You will also be instrumental in building capacity within the team, supporting colleagues through education and supervision, and working collaboratively to enhance social work practices across community mental health services. Key responsibilities include family-sensitive practice, risk assessment, community advocacy, and supporting applications for services such as NDIS, VCAT, housing, and Centrelink.

About Us

Monash Health Mental Health Services is one of Victoria’s largest mental health providers, delivering a wide range of community and specialist services across the southeastern suburbs of Melbourne.

 

The Clayton Continuing Care Team is a dedicated multidisciplinary team providing community-based mental health support to adults living with complex and enduring mental health conditions. Our team includes experienced case managers from nursing, occupational therapy, and social work backgrounds, supported by consultant psychiatrists, psychologists, peer workers, and other allied health professionals.

 

We operate Monday to Friday from our community clinic, delivering person-centred care through both in-clinic appointments and outreach into clients’ homes. The team is committed to supporting recovery through holistic, collaborative care that recognises the importance of family and carer involvement. By coordinating interventions and connecting clients to relevant community supports, we aim to empower individuals to manage their mental health and maintain independence in the community.

 

About You 

You are an experienced and highly skilled Social Worker with a minimum of seven years post-qualification experience and a strong foundation in mental health care. You bring advanced knowledge of social work theories and the ability to apply them in complex clinical settings. Your expertise includes comprehensive psychosocial assessment, advocacy, counselling, discharge planning, and navigating complex service systems such as NDIS, Child Protection, Family Violence, and the legal frameworks that support them.

 

You have strong leadership capabilities, with proven experience guiding teams through change, driving quality improvement initiatives, and mentoring others in specialised clinical practice. Your communication skills are highly developed, and you thrive both independently and as part of a multidisciplinary team. Reflective in your practice, you bring cultural sensitivity, ethical awareness, and a deep commitment to delivering safe, person-centred care to vulnerable populations.

 

Educational qualifications and licences as follows:

 

  • Bachelor of Social Work or Masters Social Work Qualifying OR Masters of Social Work, OR equivalent as assessed and approved by the Australian Association of Social Workers (AASW)
  • Eligible for ordinary membership of AASW (http://www.aasw.asn.au/)
  • Written evidence of eligibility for AASW membership for overseas qualifications is required.
  • Accredited Social Worker and/or Accredited Mental Health Social Worker: As approved by the AASW
  • Completion of a post graduate qualification in relevant field or working towards is advantageous.
