About The Role 

The Community health Nurse will function as a member of a dynamic multi-disciplinary team, to promote an integrated and holistic service. The Community Health Nurse will have excellent clinical skills, specifically related to sexual and reproductive health, to ensure the delivery of best-practice health care services. Working at outreach locations delivering sexual health education as well as at Community Health Centres supporting the two Sexual and Reproductive Health Hub clinics.   

The incumbent will support Monash Health Community Youth and Family Services to achieve targets and improve service efficiency, service coordination and the client experience, manage referrals and requests for services. 

This role requires excellent administrative, IT, organizational, communication skills, the ability to work independently and a commitment to improving the delivery of health services for patients, particularly vulnerable and at-risk women. This role involves supporting the two Sexual and Reproductive Health Hub Clinic and Youth Health Hub. Supporting Youth Health Hub Requires a 7pm finish on a Thursday evening. 

 

About You: 

  • Experience working with vulnerable people. 

  • Comprehensive assessment skills 

  • Understanding of the social model of health 

  • Ability to work with adolescent and adult cohorts 

  • Sexual and Reproductive Health expertise 

  • Excellent communication skills 

  • Self-awareness and emotional intelligence 

  • Skills in fostering relationships with external stakeholders 

  • Autonomy, workload management skills and initiative 

  • Self- directed learning relevant to role 

  • High level of Information Technology skills and knowledge. 

 

What You Need: 

  • Registered with Nursing and Midwifery Board of Australia via AHPRA 

  • Current AHPRA registration as a Registered Nurse 

  • Must meet the requirements and standards for Nurse Cervical Screening Provider Certification in Victoria.
  • Certificate in Sexual Health inclusive of Cervical Screening
  • Comprehensive Sexual and Reproductive Health (Nurse) Course
  • Australian working rights 

  • Proof of COVID vaccination
